Role of Magnetic Resonance Imaging in Locally Advanced Rectal Cancer

چکیده
This chapter will give an overview of the magnetic resonance imaging (MRI) modalities used in locally advanced rectal cancer (LARC) staging with an emphasis on the role of MRI and its significance for planning an effective therapeutic strategy for the individual patient. Specifically, the aim of this chapter is to present a brief review about: 1. Methodologies of Magnetic Resonance Imaging in LARC staging a. Morphologic MRI b. Functional MRI i. Dynamic Contrast Enhanced MRI (DCE-MRI) ii. Diffusion Weighted MRI (DW-MRI) 2. The role of MRI in diagnosis, staging, evaluation of response of neoadjuvant treatment, follow-up post surgery. This chapter will be organised in the following sections. First, in order to better define the role of MRI in LARC management, we will briefly describe the epidemiological scenario and therapeutic options, with an emphasis on issues in which MRI is relevant. Second, we will describe morphologic and functional MRI including DCE-MRI and DW-MRI. Finally, a systematic review of the literature concerning MRI, CT and PET for LARC management will be presented.

Magnetic resonance imaging for diagnosis and neoadjuvant treatment evaluation in locally advanced rectal cancer: A pictorial review
High-resolution pelvic magnetic resonance imaging (MRI) is the primary method for staging rectal cancer. MRI is highly accurate in the primary staging of rectal cancer; however, it has not proven to be effective in re-staging, especially in complete response evaluation after neoadjuvant therapy. Neoadjuvant chemoradiotherapy produces many changes in rectal tumors and on adjacent area, as a resu...

Background: This study investigated compatibility between post chemoradiation magnetic resonance images and histologic findings after operation and chemoradiation in patients with locally advanced rectal cancer. Methods: In this prospective study, 63 patients referred to Cancer Institute of Emam Khomeini Hospital, Tehran, Iran, from October 2011 to October 2013 with locally advanced rectal c...

Background The performance of TVS, TRS and MRI for the diagnosis of DIE have been separately reported in earlier studies. However, the three methods have not been concurrently compared in terms of their overall performance and classified results as per different anatomical locations of DIE lesions in a large study population. This study was an attempt to compare pelvic magnetic resonance imagin...
متن کاملOptimal imaging staging of rectal cancer
A patient diagnosed with rectal cancer is managed by a multidisciplinary team in which the radiologist nowadays participates as a full sparring partner. His/her imaging findings can influence the treatment decision-making. The local staging work-up consists of endorectal ultrasound and/or magnetic resonance imaging (MRI). The distant staging work-up depends on the local policy but often consist...
متن کاملPelvic colorectal recurrence: crucial role of radiologists in oncologic and surgical treatment options
Radical resection is the only potential cure for patients with locally advanced primary and recurrent rectal cancer and is considered curative only when the histologic margins are clear of tumour. Early diagnosis of the disease is essential as it increases the likelihood of a potentially curative resection and prevention of dissemination. Clinical examination, tumour markers and radiologic moda...
